Submit a Peer Visitor Report
Reporting your peer visit enables the Amputee Coalition to provide helpful resources to new amputees and maintain funding for this wonderful program.
All Amputee Coalition Certified Peer Visitors (CPV) are required to recertify every two years through an online recertification webinar and post-test. Background check renewals ($17.50 payable to our third party) are required every four years. For example, if you were certified or recertified in 2019 then you do not have to take the recertification test course until 2021 and your background check renewal will be in 2023.
If you do not recertify every two years, you will be required to attend one of our in-person CPV trainings. Only individuals with up-to-date credentials will be called upon for peer visits.
