Conference COVID-19 Vaccination Verification Process


Amputee Coalition will uphold its 2022 National Conference COVID-19 Vaccination Policy for all conference attendees.

Amputee Coalition places a high priority on providing and maintaining a safe work and conference environment. Following guidance from the Centers for Disease Control and Prevention (CDC), all conference attendees are required to be fully vaccinated against COVID-19 by August 1, 2022, unless they have obtained an approved exemption as a reasonable accommodation, consistent with applicable law.

Conference attendees should report their vaccination status via ID.me. Once this process is successfully completed, conference attendees should bring their email confirmation to on-site registration at the 2022 National Conference. You will be required to provide proof of your email confirmation when checking in to receive your conference badge.

If you are not vaccinated, then you are required to provide a negative COVID-19 test result within 72 hours of the conference start date. Please provide a PCR test result from a COVID Testing Center. You will be able to upload your negative test result via ID.me only within 72 hours prior to the start of the event to receive your email confirmation for successfully completing the process. Proof of your email confirmation OR negative COVID-19 test results should be provided at on-site registration when checking in to receive your conference badge.

GETTING STARTED WITH ID.ME

To update your vaccination status, simply click on the link below: https://hosted-pages.id.me/health/2022-national-conference

PLEASE NOTE, before getting started, please scan and/or take pictures with your smartphone of the front and back of your vaccination card.

Setting up the Account

  1. On the sign in page, select Create an ID.me Account.
  2. Enter an email address you can access and choose a strong password.
  3. Check the box to accept the terms of use and privacy policy, then select Create account.
  4. If prompted confirm your email address and/or set up multi-factor authentication.

Verify ID

  1. ID.me will ask you to verify your identity. You can either select photos license, state-issued ID or passport. Alternatively, you may just be asked to submit your phone number.
  2. Enter your photo number when prompted to and receive a link to your mobile phone to take pictures of your ID. (To access this option, your mobile device must have a camera and a web browser).
  3. You will be prompted to take pictures of the front and back of your state-issued ID.
  4. Once the photos are taken and uploaded, you will be asked to verify. Once you do, move back to your computer to continue the process.

Submit Vaccine Information

  1. You have the option to submit digital smart health records OR manually input information with photo of your vaccine card as verification.
  2. If you manually input your information, you will be asked for the manufacturer and date of shot(s) and booster(s)
  3. After manually inputting information, you will be asked to upload proof of vaccination. Eligible documents include:
    • COVID-19 Vaccination Record Card
    • State Immunization Registry Record
    • Health Provider Vaccination Record
  4. You will be asked to upload the front and back of your COVID-19 Vaccination record Card or submit the file of the other records. YOU WILL NOT BE PROVIDED WITH ANOTHER TEXT LINK TO SUBMIT PICTURES.
  5. Once uploading pictures, you will need to confirm the summary of information – vaccine manufacture, dates, and photos.
  6. Once verified, you will receive an email confirmation. View sample email confirmation.
  7. Bring email confirmation to on-site registration at the 2022 National Conference. You will be required to provide proof of your email confirmation when checking in to receive your conference badge.

Note: Children under the age of 18 years old cannot be verified via ID.me. A negative COVID-19 test result within 72 hours of the conference start date will be required for children under the age of 18 years old to participate in the 2022 National Conference. Please provide a PCR test result from a COVID Testing Center.  Proof of negative COVID-19 test should be provided at on-site registration when checking in to receive your conference badge.
