Submit a Peer Visitor Report
Reporting your peer visit enables the Amputee Coalition to provide helpful resources to new amputees and maintain funding for this wonderful program.
All Amputee Coalition-certified peer visitors are required to recertify, but certification is now valid for two years. So, if you were certified or recertified in 2015 then you do not have to take the course until 2017.
If you were not certified or did not recertify in 2015, then you must take the recertification course and post-test to maintain your status as an active peer visitor. Only individuals with up-to-date certification will be called upon for peer visits.
If you are not sure if you are required to recertify this year or if you have any other questions, please contact email@example.com.
