Peer and Parent Peer Visitation Reporting Forms

The Amputee Coalition is proud to offer a way for those involved in peer visitation (either as an individual or through a local amputee support group) to:

  • Add those they visit to the Amputee Coalition mailing list to receive the inMotion magazine and other information
  • Submit requests for further information on behalf of the new amputee
  • Submit information about the visit electronically, by mail, or by fax

This section of the site has frequently asked questions that are related to peer visitation, both on behalf of the new amputee and the peer visitor.  If you have been asked a tough question that you weren't sure how to address, or if, as a peer visitor, you have questions you would like an answer to, this is the place to find them.  If you can't find the answer to your question on this page, please submit your question electronically.

Peer Visitors Discussion BoardNew!

The Peer Visitor Discussion Board is a tool specifically for Peer Visitors to use whenever they have issues that surface from the Peer Visitation process.  Registered users can view and post topics about questions that only another PV could answer or to discuss emotions evoked from visits.  Whatever your needs, this is a community that will help support you as long as comments and conversations are respectful.
