Amputee Communicator Forum

How to Register:

  1. Review the following criteria and confidentiality information before registering to participate in the Amputee Communicator Forum.
  2. Click here to download and print the Registration and Physician Confirmation Forms. Note these forms are in PDF format and require Acrobat Reader, available FREE from Adobe.
  3. Complete and mail the forms to the Amputee Coalition at the address indicated on the forms.

Once the documents have been received by the Amputee Coalition, you will be contacted via email with the information you need to access the Forum.

 

Criteria for Participation

  • Participants need a computer with internet access.
  • Participants must complete the Registration Form and the Physician Confirmation Form. The forms must be completed and returned to the Amputee Coalition at the address indicated on the forms.
  • Participants must be an amputee and at least 18 years of age.
  • Understand that the Amputee Communicator Forum is meant to be an informational and educational support program for amputees.

Confidentiality and Privacy

It would be impossible for Amputee Coalition to enforce confidentiality among participants of the Amputee Communicator Forum. So, we turn to you and ask that you respect other Forum members and refrain from sharing any personal information or knowledge that is revealed in the postings. We are counting on your integrity and devotion to your fellow amputees to prevent any gossip. The goal of the Amputee Communicator Forum is to provide a safe, comfortable atmosphere where all participants feel free to share the deepest of feelings and concerns. It is, however, up to you to maintain and ensure that everyone has that experience.

It is Amputee Coalition's policy that personal contact information that may be collected from users of the Amputee Communicator Forum will not be sold or otherwise provided to third party organizations or used in commercial activities. Any demographic information requested is used solely to assist the Amputee Coalition in carrying out its missions of outreach, empowerment, and education. All personal information is held in strict confidentiality.
