Hospital Membership without Peer Support Program Benefits

If you’re interested in participating in the Amputee Coalition’s Peer Visit Program which meets CARF certification standards, you must be a hospital member and an additional contract would need to be signed outlining the program.

Publications:

  • Every Hospital Member facility receives one complementary issue of inMotion
  • Every Primary Hospital member facility receives 50 sign up cards for patients to register for inMotion for FREE
  • Every Affiliate Hospital member facility receives 25 sign up cards for patients to register for inMotion for FREE
    • Each Primary Hospital Member will receive 50 copies of each of the following for free

Membership Costs:
Primary Hospital Membership Price:  $1,000.00 per year
Affiliate Hospital Membership Price: $300 per Facility per year after Primary Facility

*Note – only Hospitals are eligible to purchase a Hospital Membership.  Hospital memberships with the Amputee Coalition are on a calendar year basis.  In order to be a member with the Amputee Coalition, one of your Hospital facilities must hold a Primary Hospital Membership.  Additional Affiliate Memberships are available to purchase for the reduced rate and provide the benefits outlined above.
