Please understand that the Amputee Coalition cannot and does not make specific recommendations for products or services, but can only suggest possible sources, with no endorsement implied.

Amputee Coalition therefore does not maintain a list of physical therapists experienced with amputation. The following Internet search engines can help you find physical therapists and physiatrists near you. You can then contact these professionals to ask about their experience and specialties.

Amputee Coalition Member Corporate, Facility and Hospital Directory

The first resource we suggest is the Amputee Coalition Member Corporate, Facility and Hospital Directory (http://www.amputee-coalition.org/members/members.asp), where you can search for facilities by state or within a certain distance of a ZIP code. Not all search results will be rehabilitation centers: Some will be orthotic & prosthetic facilities, hospitals, or prosthetics manufacturers, but all have demonstrated their commitment to the limb loss community by becoming members of the Amputee Coalition.

American Physical Therapy Association

1111 North Fairfax Street
Alexandria, VA 22314-1488
800/999-2782

On this Web page, you can search for member physical therapists within 10 to 50 miles of a given ZIP code. You can also select an area of expertise (Orthopaedics).

American Academy of Physical Medicine and Rehabilitation

Chicago, IL 60611-7617
312/464-9700

On this Web page, you can select a state for a list of academy-member physiatrists (physicians specializing in physical medicine and rehabilitation) listed alphabetically by city in that state, then select names from the list for contact information. No information on particular areas of expertise is given.

The CARF Web page states, “For a list of service providers in your region with CARF-accredited Amputation Programs, please contact medical@carf.org.” Or, call CARF Amputation Specialty Programs at 888/281-6531.

Case Management Resource Guide

The online version of the official directory of the Case Management Society of America offers a search for service providers within 5 to 100 miles of a given ZIP code. Among the categories offered are Physical & Occupational Therapy Services, Prosthetic & Orthotic Services, and Rehabilitation Facilities.
