College Scholarship Available for Amputees

U.S. residents under the age of 35 who are current or former cancer patients OR an amputee are urged to apply for the Cancer for College scholarship.

 

Submit your name, address, date of birth, telephone number, e-mail address, expected college entry date, the name and address of college(s) you are attending or applying to, field of desired study and a chronological list of any activities you have participated in during the last four years.

 

The following information must also be included:

·        Summary of cancer treatment that includes date of diagnosis OR a summary of condition that resulted in amputation. Both summaries must include place of treatment, status of treatment and doctor’s contact information.

·        A personal statement explaining why you should be selected as a scholarship recipient.

·        Description of other sources you are using to finance your college education.

·        Two letters of recommendation from people outside your immediate family (e.g., doctors, nurses, teacher). 

 

The deadline for this annual award is September 1, 2006.  Applicants received after this date will be considered for the 2007 award.  Please contact cancerforcollege@hotmail.com with questions or for further information.
