The ACA’s online survey for January is designed to determine the impact of providing care for someone with limb loss. Responses to the survey will help the ACA identify caregiver needs and develop articles and fact sheets to address those needs.

Web-based surveys enable the ACA to gather information from the amputee community, which is useful for assessing needs and planning or evaluating program activities. The “user surveys,” found on the lower left corner of each Web page, change frequently and take no more than 2 minutes to complete. Results of the surveys are updated with each completed survey and are immediately viewable. Web users can return to the site at a later date to view ongoing changes in the results.

Other surveys may be found by clicking on the “Surveys” button on the upper left menu. These surveys have the same viewing and ease-of-use features but may take 3-5 minutes to complete.

Please take a few minutes each month to participate in one or more of our surveys. Who you are, your needs, and your opinions are important. You may access surveys by going to www.amputee-coalition.org.

Founded in 1986, the Amputee Coalition of America (ACA) is a national 501(c)(3) nonprofit organization headquartered in Knoxville, Tennessee. The ACA’s mission is to reach out to people with limb loss and to empower them through education, support and advocacy. The ACA is partially supported by cooperative agreement No. E11/CCE419270 from the National Center on Birth Defects and Disabilities, Centers for Disease Control and Prevention (CDC). The contents of this message are solely the responsibility of the authors and do not necessarily represent the official views of the CDC, the sponsoring organizations or the ACA. No funding from the CDC is used to support ACA advocacy efforts.
