Whether you’re an amputee or a friend or relative of an amputee, in the time it takes to click your mouse twice you can help the Amputee Coalition of America (ACA) determine how insurance coverage has changed for people with private insurance.

Completing the user survey on the bottom left of the ACA home page (www.amputee-coalition.org) will provide data that will support the ACA’s grassroots and advocacy efforts for all amputees in the United States.

We’d love to hear your story too! Just click on the button below the survey to tell us about your experience with insurance coverage. What have you faced? What are your challenges?

For more information on how the ACA can help you, please contact us toll-free at 888/AMP-KNOW (888/267-5669) or by e-mail at acainfo@amputee-coalition.org.
